How to Apply Online:
Step 1: Visit the official website:
Go to to start the application process.
Step 2: Register:
Select ‘Register’ option. Enter your username, mobile number and email id.
You will be redirected to the portal based on your state (Karnataka, Telangana, Maharashtra, Tamil Nadu etc.). Click on the appropriate link.
Step 3: Register on State Portal:
Click ‘Register’ again. Fill in the details like name, last name, gender and date of birth. Click ‘Next’ to continue.
Step 4: Enter Address Details:
- State
- District
- Sub-District
- Village/City
- Pincode
- Building and House No
- Name of the road
- Click ‘Next’ when done.
Step 5: Give Aadhaar and National Information:
Enter your Aadhaar Number and select Nationality. Tick the consent box and click ‘Next’. Enter your mobile number to get OTP for verification.
Step 6: Verify Email ID:
After entering the mobile OTP, give your email id. Click ‘Skip and Register’ to go to the login page.
Step 7: Login:
Enter your registered mobile number and captcha. Get OTP and enter to access your account.
Step 8: Report the birth:
Click the three-line menu at the top right. Select “Birth” and select “Report Birth”.
Now your registration process starts
Step 1: Fill in birth details: Select the state you were born in (eg, Karnataka). Select the default language (English) and optionally, Hindi. Registration date will be generated automatically.
Step 2: Enter the child’s information: Give the date and time of birth. Select gender. Enter the child’s Aadhaar number, if available.
Step 3: Name Entry: If the child’s name is not yet final, tick the appropriate box. Otherwise, enter first and last name.
Step 4: Enter Parent Information: Give Father’s Details (Name, Last Name, Aadhaar Number, Email, Mobile Number). Also give details of mother.
Step 5: Fill in the address details: Select ‘India’ as the address location. Enter the parent’s address or tick ‘Copy Parent’s Address’ if applicable.
Step 6: Select Place of Birth:
Select the place of birth (eg, hospital, home, etc.). Select State, District and Sub-District. While selecting the city, give the ward details.
Step 7: Select Registration Section: Select ‘Registration Section’ and hospital name. If the hospital is not listed, type it in yourself.
Providing statistical information (fill in details):
- Residential addresses of parents (if different from each other).
- Father’s religion, education and occupation.
- Health details of mother at birth.
- Number of children the mother has before this birth.
- Delivery details (type of hospital, mode of delivery, birth weight of baby, duration of pregnancy).
Uploading the required documents
You must upload the following documents (each should be less than 8MB):
Hospital discharge certificate
Proof of Identity (PAN Card, Aadhaar etc.)
Government approved birth certificate endorsement
